Please note that:

• When you submit the form you will get an email confirmation for your own records
• You won't be able to subsequently amend/edit your record (a member of LMA/Lloyd's may do this for you if needed)
• You can view a summary of all the responses about each coverholder/DCA by your Managing Agency, irrespective of who provided the feedback, in the table below
